If you are visiting us for the first time, please bring the following:
- Any X-rays or MRIs taken for your current pain problem.
- Any report(s) from other doctors about this problem
- Location specific patient intake paperwork
- Insurance cards and photo ID
If you are not able to print the new patient paperwork, you can complete it when you arrive at our office.
