To schedule an appointment, please have your primary care or referring physician send us a referral request. If you would like to self-refer, please contact one of our three clinics and the staff will be happy to help you.

If you cannot keep an already scheduled appointment, please contact the clinic directly as soon as possible. Appointments that are cancelled with less than 24 hours’ notice are subject to a $50 cancellation fee which cannot be billed to insurance.
